Your passport is going to take 4-6 weeks. You can pay extra to have it expedited. Go to almost any U.S. Post Office for passport.

How soon do you have to go?
You can get the passport application and instructions at the State Dept website. For an extra fee, you can get it in two weeks. If you are leaving within the week and have your ticket already, they can do a same day passport for you, but you will have to go to a passport office (there are only a few in every state)
For the visa, contact the Indian embassy. Same, they can do a one day for you but you need to go the nearest consulate.
